Duties

• Monitor patient observations, including vital signs and overall condition, to ensure safety and health status.
• Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support.
• Administer medications accurately following medical documentation protocols and home healthcare regulations.
• Support patients with dementia, Alzheimer's, memory care, and other cognitive impairments through specialized care techniques.
• Operate Electronic Health Record (EHR) systems to document medical records, treatments, and patient progress efficiently.
• Provide catheter care, wound care, and hospice/end-of-life support with sensitivity and professionalism.
• Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a clean and safe environment.
• Use medical terminology appropriately when recording notes or communicating with healthcare teams.
• Support patients during transfers using equipment like Hoyer lifts or other assistive devices while practicing proper body mechanics for heavy lifting safety.
• Follow HIPAA guidelines to ensure patient confidentiality at all times.
• Respond promptly to patient needs during home health visits or in long-term care settings.
