Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and equipment such as Hoyer lifts.
• Monitor patient health by checking vital signs, observing for changes in condition, and reporting findings accurately to healthcare teams.
• Prepare nutritious meals aligned with dietary restrictions and preferences, ensuring proper food safety and hygiene standards.
• Monitor medication reminders safely following care plans and HIPAA regulations, maintaining detailed documentation of medication administration.
• Support clients with cognitive impairments such as dementia or Alzheimer's disease through memory care strategies and specialized activities.
• Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment to promote overall health and comfort.
• Document all care activities meticulously, including patient observations, progress notes, and adherence to individualized care plans.

Skills

• Proven experience working in group homes, assisted living facilities, nursing homes, hospice or home health environments.
• Strong patient observation skills with the ability to recognize subtle changes in condition or behavior.
• Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information.
