Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature
• Perform specialized care tasks such as catheter care, hospice support, dementia and Alzheimer’s care, and end-of-life assistance
• Utilize Electronic Health Record (EHR) systems to document patient progress, medical records, and care activities efficiently and securely in accordance with HIPAA standards
• Support patients with memory care needs by implementing personalized routines and ensuring a safe environment
• Operate medical equipment including Hoyer lifts and other assistive devices to promote patient safety during transfers and mobility tasks
• Prepare nutritious meals, assist with feeding when necessary, and maintain cleanliness through light housekeeping duties such as laundry and cleaning to ensure a healthy living space
• Provide emotional support and companionship to foster a positive environment for clients with developmental disabilities or geriatrics

Experience

• Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred
• Demonstrated experience caring for geriatric patients or individuals with disabilities, including dementia or Alzheimer’s disease
