Duties

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and manual handling techniques.
  • Take and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely reporting to nursing staff.
  • Support medication administration processes in compliance with HIPAA regulations and medical documentation standards.
  • Provide specialized care for individuals with dementia or Alzheimer's disease, ensuring their safety and comfort through memory care techniques.
  • Collaborate with interdisciplinary teams in diverse settings such as hospitals (including ICU and trauma centers), long-term care facilities, assisted living communities, hospice care environments, and outpatient clinics.
  • Document all patient interactions thoroughly within Electronic Health Record (EHR) systems like CareBridge.
  • Must be able to show up to work.

Skills

  • Proven experience working with geriatric patients in medical-surgical or long-term care environments; familiarity with senior care and home & community care is highly valued.
  • Must have a car that is dependable.
