Job Overview

Duties

• Observe and monitor patients’ conditions regularly, documenting changes accurately using PointClickCare (POC) or Electronic Medical Record (EMR) systems.
• Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Administer medications as prescribed, ensuring proper documentation in compliance with home health regulations and HIPAA standards.
• Perform vital signs assessments such as blood pressure, pulse, respiration, and temperature to track patient health status.
• Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management.
• Support patients during end-of-life or hospice care with compassion, dignity, and respect for their wishes.
• Handle catheter care, wound dressing changes, and other medical procedures under supervision or according to training protocols.
• Assist with light housekeeping duties such as cleaning patient areas and laundry to maintain a safe and hygienic environment.
• Prepare nutritious meals tailored to dietary restrictions and assist with meal consumption when necessary.
