Duties

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using safe lifting practices and equipment like Hoyer lifts.
• Take and record vital signs including blood pressure, temperature, pulse, and respiration rates with accuracy and professionalism.
• Support medical procedures by preparing patients, maintaining aseptic techniques, and assisting healthcare professionals during treatments such as catheter care or medication administration.
• Document patient information thoroughly using Electronic Health Records (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations and medical documentation standards.
• Monitor patients’ conditions closely, especially those with complex needs such as dementia or memory care residents, providing attentive observation for any changes requiring escalation.
• Facilitate communication between patients and healthcare teams to ensure clear understanding of care plans and patient needs.
• Support discharge planning or transition to home & community care by educating patients and families on post-care instructions.
