Job Summary

We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to join our dynamic two-bedroom hospice home in Penfield, NY. In this special role you will provide compassionate resident end-of-life care in a home environment. Shepherd Home operates under the hospice philosophy, providing 24-hour care for the terminally ill in a residential setting. Shepherd Home offers physical, emotional and spiritual support to those facing the prospect of death. We also provide hospitality and support to family members and friends as well as bereavement counseling and community education.

Responsibilities

• Provide compassionate overnight care and supervision while remaining awake, alert, and attentive throughout the shift
• Provide specialized, non-curative care to residents facing terminal illness in a comfort care home setting
• Focus on symptom management, pain relief, personal care, and promoting comfort, dignity, and quality of life
• Provide compassionate emotional support and hospitality to residents, families, and friends throughout the end-of-life journey
• Observe and communicate changes in residents condition and report concerns promptly to the Nurse Supervisor
• Work in closely and in harmony with team volunteers for exceptional delivery of care

Skills

• A minimum of 1 year experience (hospice, oncology, medical-surgical, long-term care or home health care preferred)
• Personal Care to include assisting with toileting, oral care, bedside commodes, and other personal care as needed
• Clinical support as trained and supervised under the direction of the nurse coordinator
• Strong communication, common sense, and problem solving skills
• Empathy, compassion, and emotional intelligence skills are a must
• Excellent time management and organizational skills
• Proven ability to work both independently and in a team environment
