Job Summary

We are seeking a dedicated and compassionate Home Health Aide/Certified Nursing Assistant to deliver exceptional in-home care to our clients. The ideal candidate will possess a strong background in patient care, with experience working with diverse populations including seniors, individuals with disabilities, and those requiring specialized dementia or hospice care. This role involves assisting with daily activities, monitoring health status, and ensuring a safe and supportive environment for our clients.

Responsibilities

• Provide personal care including bathing, grooming, dressing, and continence management while respecting client dignity.
• Assist with activities of daily living such as meal preparation, light housekeeping, laundry, and transportation to appointments or social outings.
• Monitor vital signs and observe patient conditions to report any changes or concerns promptly.
• Support clients with mobility assistance using equipment like Hoyer lifts or transfer belts to promote safety.
• Administer medications and perform basic medical tasks such as catheter care under supervision or as directed by healthcare professionals.
• Maintain accurate medical documentation within Electronic Health Record (EHR) systems while adhering to HIPAA regulations.
• Engage clients in companionship activities and life enrichment exercises to promote mental well-being.
