Overview

We are seeking a dedicated Home Health Aide/Certified Nursing Assistant to join our team. This role offers an opportunity to make a meaningful difference in the lives of clients by delivering exceptional personal and medical care within their homes. The ideal candidate will possess a strong commitment to patient well-being, excellent communication skills, and experience working with diverse populations, including seniors and individuals with disabilities.

Responsibilities

• Provide direct patient care including assistance with activities of daily living such as bathing, dressing, grooming, and toileting.
• Monitor and observe patients’ health status, recording vital signs and reporting any changes to supervising nurses or care managers.
• Assist with medication reminders and medication administration following home healthcare regulations and medical documentation standards.
• Support patients with mobility using assistive devices like Hoyer lifts or transfer belts while ensuring safety protocols are followed.
• Perform light housekeeping duties such as laundry, cleaning, meal preparation, and ensuring a safe environment by eliminating tripping hazards.
• Document patient care activities accurately using electronic health record (EHR) systems or medical records management tools.
• Provide companionship and emotional support to promote mental well-being, especially for clients with dementia or Alzheimer’s disease.

Qualifications

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
