Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and document patient vital signs such as blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems
• Observe patient conditions closely, noting any changes in behavior or health status for timely reporting
• Support medication administration following proper medical terminology and home healthcare regulations
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and dementia-specific activities
• Maintaine comfort and safety
• Prepare meals, assist with feeding when necessary, and maintain cleanliness of patient living areas including laundry and general cleaning tasks

Experience

• Prior experience working in assisted living facilities or nursing homes is highly preferred
• Demonstrated experience caring for geriatric patients or individuals with developmental disabilities
• Knowledge of medical terminology, medication administration procedures, and HIPAA privacy standards
• Hands-on experience with electronic time login and documentation
• Familiarity with home health agency operations, home & community care regulations, and use of equipment such as Hoyer lifts
