We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients with diverse needs. The ideal candidate will possess a strong commitment to patient-centered care, with experience in assisting with activities of daily living (ADLs), medical documentation, and working within home health or long-term care environments. As a vital member of our caregiving team, you will deliver personalized support that promotes independence, safety, and well-being for individuals across various stages of health and mobility. Duties

• Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
• Monitor and observe patients’ conditions, accurately documenting vital signs, medication administration, and changes in health status using electronic health record (EHR) or electronic medical record (EMR) systems.
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques.
• Support clients in assisted living settings or their private homes by preparing nutritious meals, maintaining cleanliness, and managing laundry tasks.
• Administer medications following prescribed protocols while ensuring compliance with HIPAA regulations to protect patient privacy.
• Assist with medical procedures such as catheter care, hospice support, and wound management under the supervision of nursing staff.
• Engage with patients respectfully and empathetically, fostering a positive environment that encourages dignity and independence.
• Collaborate effectively with healthcare professionals to ensure comprehensive patient care and accurate medical documentation.

Qualifications

• Must be a certified Home Health Aide (HHA) or Personal Care Aide (PCA).
• Must speak English fluently.
