Overview

We are seeking compassionate and dedicated Home Health Aide (HHA) Certified Nurses Aide (CNA)Personal Care Assistant (PCA) Homemaker (HMK) / Companion to provide essential support and companionship to individuals in their homes. This role offers an opportunity to make a meaningful difference in clients' lives by assisting with daily activities, ensuring safety, and promoting independence. The ideal candidate will possess a strong background in patient care, home health services, and working with diverse populations, including seniors and individuals with disabilities or chronic conditions. This position is vital for fostering a safe, comfortable, and engaging environment for clients while adhering to all healthcare regulations and standards.

Duties

  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
  • Monitor and observe patients' health status by reporting changes, and maintaining accurate medical documentation within Notes /Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
  • Support medication management by reminding patients to take their medication and ensuring compliance with HIPAA regulations for patient privacy.
  • Provide companionship to reduce feelings of loneliness or isolation, engaging clients in conversation, activities, or hobbies suited to their interests.
  • Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with infection control standards.
  • Support clients with specialized needs such as dementia care, Alzheimer's care, hospice support, or working with individuals with developmental disabilities or disabilities.

Skills
