Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patient health status, recording vital signs and noting any changes in condition
• Administer medications and perform basic medical procedures such as catheter care and wound care under supervision
• Support patients with memory care needs, including Alzheimer’s and dementia care, ensuring a safe environment
• Utilize Electronic Health Record (EHR) systems to document patient interactions, medical records, and care plans accurately
• Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
• Follow home healthcare regulations and HIPAA guidelines to maintain patient privacy and safety standards
• Assist with light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment
• Operate medical equipment like Hoyer lifts or other assistive devices safely for patient transfers
• Collaborate with nursing staff and other healthcare professionals to deliver comprehensive patient care

Skills

• Proven experience working with individuals with disabilities or in assisted living environments
