Home Health Aide

Job Summary

We are seeking a compassionate, dependable, and dedicated Home Health Aide (HHA) to join our caregiving team. In this role, you will provide essential in-home support to elderly, disabled, or recovering patients, helping them maintain their independence and dignity within the comfort of their own residences. You will work under the supervision of a registered nurse to execute personalized care plans that focus on both physical assistance and emotional well-being. We are hiring in multiple locations Medina, Chardon, Geauga. Lake and surrounding counties ​

Key Responsibilities

• Personal Care Assistance: Assist clients with daily activities (ADLs), including bathing, grooming, dressing, toileting, and oral hygiene.
• Health Monitoring: Measure and record vital signs such as pulse, temperature, and blood pressure, reporting any significant changes to the supervising nurse.
• Medication Support: Provide medication reminders or assist with self-administered medications according to the prescribed care plan.
• Mobility & Transfers: Help patients move safely, including transfers from beds to wheelchairs and assisting with prescribed range-of-motion exercises.
• Nutritional Support: Plan and prepare nutritious meals based on specific dietary restrictions and assist with feeding when necessary.
• Homemaking Tasks: Perform light housekeeping duties such as laundry, vacuuming, dusting, and maintaining a clean, safe environment.
• Transportation & Errands: Accompany or transport patients to medical appointments, grocery shopping, or other essential community outings.
• Companionship: Offer emotional and psychological support through meaningful conversation and social engagement to reduce isolation.

Requirements & Qualifications

• Education: High school diploma or equivalent (GED).
