Job Summary

Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Monitor patient health by observing vital signs, assessing symptoms, and reporting changes promptly using Electronic Health Record (EHR) systems
• Administer medications accurately following home healthcare regulations and medical documentation standards
• Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management
• Support end-of-life and hospice care needs with compassion, dignity, and adherence to HIPAA guidelines
• Perform catheter care, wound management, and other medical procedures with proper technique and safety protocols
• Assist with meal preparation, light cleaning, laundry, and maintaining a safe home environment to promote overall well-being
• Document all patient interactions thoroughly in medical records while ensuring confidentiality and compliance with HIPAA regulations
• Utilize EMR systems efficiently for scheduling, documentation, and communication within the home health agency

Qualifications
