Shifts hours 4pm-7am Saturday & Sunday

Duties

• Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote independence and dignity
• Monitor patients’ health by observing vital signs, reporting changes, and maintaining detailed care plans in compliance with HIPAA regulations
• Prepare nutritious meals tailored to dietary restrictions and preferences, ensuring proper nutrition and hydration
• Administer medications accurately according to prescribed schedules and document all medication-related activities
• Support mobility using equipment like Hoyer lifts or assistive devices; perform heavy lifting when necessary to ensure safety during transfers
• Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces
• Provide companionship and emotional support to reduce feelings of loneliness or isolation while engaging clients in meaningful activities

Skills

• Proven experience in home & community care settings, assisted living facilities, nursing homes or group homes
• Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and working with individuals with developmental disabilities or autism
