HHA/Caregiver

Overview

We are seeking a compassionate and dedicated Home Health Aide (HHA)/Caregiver to provide exceptional in-home care to individuals with diverse needs. The ideal candidate will possess a strong commitment to patient well-being, with experience in assisting with activities of daily living (ADLs), medication management, and providing emotional support. Duties

• Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support, including heavy lifting and the use of assistive devices like Hoyer lifts.
• Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR).
• Administer medications following prescribed schedules and ensure compliance with home healthcare regulations and HIPAA privacy standards.
• Support clients with specialized care needs including Alzheimer’s care, dementia care, memory care, hospice care, catheter care, and end-of-life assistance.
• Provide companionship and emotional support to individuals with developmental disabilities or geriatric patients to promote mental well-being.
• Prepare nutritious meals aligned with dietary restrictions and assist with cleaning, laundry, and light housekeeping tasks to maintain a safe and comfortable environment.
• Document all patient interactions thoroughly in medical records, ensuring accuracy for ongoing care coordination.
• Collaborate effectively with healthcare professionals to ensure comprehensive patient care plans are followed.
