LPN - Assisted Living- Night Shift - *7p-7a*

Join our dedicated healthcare team as an Assisted Living LPN.

In this vital role, you will provide compassionate nursing care for our seniors living in assisted living facilities. Your clinical skills, combined with a nurturing approach, will ensure residents receive safe, effective, and personalized care. Responsibilities

• Conduct patient assessments, including vital signs, physical examinations, and medication reconciliation to develop individualized .
• Administer medications, injections, and more following strict safety protocols and HIPAA regulations.
• Monitor residents’ health status using our EMR (Electronic Medical Records) system, Point Click Care, to document all medical records accurately and promptly.
• Collaborate with interdisciplinary teams to coordinate behavioral health support, dementia care strategies, and long-term care management.
• Provide specialized care for residents with complex needs such as memory care, Alzheimer's disease.
• Ensure infection control practices are rigorously followed to maintain a safe environment and prevent healthcare-associated infections.
• Supervise and mentor nursing assistants and team members to promote best practices in patient care and uphold regulatory standards including state healthcare regulations and long-term care regulations.

Requirements

• Valid LPN/LVN license with current registration in the state.
• Experience in assisted living facilities or long-term care settings; experience with dementia care and behavioral health is highly desirable.
• Familiarity with EMR/EHR system, Point click Care to ensure accurate patient data management.
• Ability to lead a team efficiently while demonstrating leadership.

Pay: $24.00 - $32.00 per hour
