Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health through observation of vital signs and overall condition, promptly reporting any concerns
• Administer medications and perform catheter care following medical instructions and home healthcare regulations
• Support clients with memory care needs, including dementia and Alzheimer's care, ensuring safety and comfort
• Document patient information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Aid with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and hygienic environment
• Use patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility and transfers

Qualifications

• Prior experience in assisted living, nursing homes, or home healthcare settings preferred
• Knowledge of medical terminology, medication administration, and vital signs monitoring
• Experience working with individuals with developmental disabilities or dementia preferred
• Certification in CPR and First Aid required; Basic Life Support (BLS) certification highly desirable
• Familiarity with HIPAA regulations and maintaining patient confidentiality essential
