Responsibilities

• Provide compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) systems
• Assist with medication administration following prescribed schedules and medical terminology guidelines
• Perform vital signs assessments such as blood pressure, pulse, temperature, and respiration rates to ensure patient stability
• Support clients with specialized needs such as Alzheimer's care, dementia care, hospice care, and working with individuals with developmental disabilities or disabilities
• Help with medical procedures including catheter care, wound care, and end-of-life comfort measures while adhering to home & community care regulations and HIPAA privacy standards
• Assist with household tasks like cleaning, laundry, meal preparation, and ensuring a safe environment for long-term or live-in employment situations
• Use equipment such as Hoyer lifts or other mobility aids safely to prevent injury during transfers or heavy lifting tasks
• Maintain detailed medical documentation and records to ensure compliance with healthcare standards and legal requirements
