Responsibilities

  • Assess residents’ physical conditions, including mobility, strength, balance, and functional abilities, to develop tailored treatment plans.
  • Implement evidence-based physiotherapy interventions such as therapeutic exercises, trigger point therapy, and gait training to promote recovery and independence.
  • Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across inpatient, outpatient, home health, and community settings.
  • Maintain accurate clinical documentation in accordance with healthcare standards and electronic health records (EHR) systems.
  • Educate residents and their families on injury prevention, activity modifications, and ongoing care strategies.
  • Monitor progress regularly and adjust treatment plans to meet evolving patient needs while adhering to clinical documentation standards.
  • Participate in outpatient clinic or rehabilitation center activities, including assessments for orthopedics, neurology, pediatrics, and geriatrics.

Qualifications

  • Valid state licensure as a licensed Physical Therapist with practicing in a physical therapy role.
  • Proven experience working with geriatric patients in assisted living or long-term care environments.
