Certified Home Health Aide

Job Summary

We are seeking a compassionate and dedicated Home Health Aide to provide essential support to individuals in their homes or community settings. The ideal candidate will have experience in patient care, assisting with activities of daily living (ADLs), and a strong understanding of medical terminology. This role requires a commitment to enhancing the quality of life for our clients, particularly those with developmental disabilities, dementia, or Alzheimer's disease.

Responsibilities

• Assist clients with personal care tasks such as bathing, grooming, and dressing.
• Support clients in daily living activities including meal preparation, laundry, and light cleaning.
• Monitor and document vital signs and changes in client health status using EMR or EHR systems.
• Administer medications as prescribed and ensure compliance with medication schedules.
• Provide companionship and engage clients in meaningful activities to promote mental well-being.
• Assist with mobility and transfers using equipment such as Hoyer lifts when necessary.
• Maintain accurate medical documentation in accordance with HIPAA regulations.
• Collaborate with healthcare professionals to develop personalized care plans tailored to each client's needs.
• Provide specialized care for clients requiring hospice support or memory care services.

Qualifications

• Previous experience in home health, assisted living, nursing homes, or long-term care settings is preferred.
• Knowledge of patient observation techniques and ability to recognize changes in client conditions.
• Familiarity with medical terminology and procedures related to caregiving.
• Certification in CPR is highly desirable.
• Experience working with individuals with developmental disabilities or dementia is a plus.
• Strong communication skills and the ability to work effectively with clients, families, and healthcare teams.
