Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems
• Support medication administration and ensure proper documentation in compliance with HIPAA regulations
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory-related conditions, including memory care techniques
• Perform patient observation to detect changes in condition or behavior and communicate findings promptly to healthcare professionals
• Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice care support as needed
• Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, and meal preparation
• Document all patient interactions and medical records thoroughly using Electronic Medical Records (EMR) systems to ensure accurate medical documentation
