Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts
• Monitor and observe patients’ conditions, recording vital signs and medical observations accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Administer medications following medical documentation protocols while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards
• Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care and behavioral support strategies
• Support patients with catheter care, wound dressing changes, and end-of-life care with compassion and professionalism
• Assist with meal preparation, light housekeeping, laundry, and cleaning to ensure a safe and comfortable living environment
• Communicate effectively with clients, families, and healthcare teams to coordinate care plans and update medical records regularly

Experience

• Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
