Duties

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility
• Monitor patient health by observing vital signs and reporting changes promptly using Electronic Health Record (EHR) systems
• Support medication administration following medical instructions and ensuring proper documentation in compliance with home & community care regulations
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care strategies
• Perform catheter care, wound care, and hospice/end-of-life support with sensitivity and professionalism
• Help with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
• Utilize medical terminology accurately when documenting patient records and communicating with healthcare teams
• Operate assistive devices like Hoyer lifts safely to aid mobility while preventing injury
• Follow HIPAA guidelines to maintain patient confidentiality at all times
• Engage in ongoing training on medical procedures like CPR, first aid, basic life support (BLS), and home health best practices

Experience

• Prior experience working in assisted living facilities or nursing homes is highly desirable
