Duties

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
• Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within EMR (Electronic Medical Record) systems.
• Provide medication reminders and assist with medication administration following home healthcare regulations and safety protocols.
• Support patients with cognitive impairments such as dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement.
• Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment aligned with client care plans.
• Document patient interactions thoroughly while maintaining strict adherence to HIPAA regulations to protect patient confidentiality.
• Communicate effectively with clients, family members, and healthcare providers to coordinate care plans and ensure seamless service delivery.

Qualifications

• Proven experience working with individuals with disabilities, developmental disabilities, or in geriatrics settings such as nursing homes or assisted living communities.
