Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or transfer belts.
• Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately.
• Administer medications following prescribed schedules and document all medical activities in Electronic Health Record (EHR) systems while maintaining HIPAA compliance.
• Provide specialized care for clients with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies.
• Support clients in assisted living or long-term care settings by helping with meal preparation, light cleaning, laundry, and household tasks to ensure a safe and comfortable environment.
• Observe patient conditions closely, report changes promptly to healthcare professionals, and document medical records thoroughly for ongoing care coordination.

Qualifications

• Proven experience in home health care, nursing homes, assisted living facilities, or working with individuals with developmental disabilities or disabilities.
