Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patient conditions, recording vital signs such as blood pressure, pulse, temperature, and respiration accurately
• Administer medications and manage medical documentation in compliance with home health regulations and HIPAA standards
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory-related conditions, including memory care techniques
• Support patients with catheter care, wound management, hospice care, and end-of-life support as needed
• Operate Electronic Health Record (EHR) systems to document patient progress and medical records efficiently
• Assist with light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment using proper lifting techniques like Hoyer lifts when necessary

Qualifications

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in accordance with state requirements
• Proven experience working in home healthcare settings or assisted living environments
