Duties

• Observe and monitor patients’ conditions regularly, documenting changes accurately in electronic health record (EHR) systems while maintaining strict HIPAA compliance.
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary.
• Administer medications and treatments as prescribed, ensuring proper documentation and adherence to home healthcare regulations.
• Perform vital signs assessments such as blood pressure, pulse, respiration, and temperature to monitor health status.
• Support patients with specialized care needs including Alzheimer’s disease, dementia, hospice, memory care, and end-of-life care.
• Provide assistance with catheter care, wound care, and other medical procedures under supervision.
• Help with meal preparation, feeding assistance, light housekeeping, laundry, and maintaining a clean environment for residents’ safety and comfort.
• Work closely with interdisciplinary teams to coordinate patient care plans while respecting patient rights under HIPAA guidelines.
• Use EMR (Electronic Medical Records) systems efficiently for documentation of patient interactions and medical records.
• Engage in ongoing training on medical terminology, basic life support (BLS), CPR certification, first aid procedures, and home & community care best practices.
