Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patient conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) systems
• Administer medications and perform basic medical tasks such as vital signs measurement, catheter care, and wound management under supervision
• Support patients with specialized needs such as Alzheimer’s care, dementia care, hospice, and end-of-life care with patience and empathy
• Assist with meal preparation, feeding, light housekeeping, laundry, and maintaining a clean and safe environment
• Use assistive devices like Hoyer lifts or other mobility aids to ensure safe transfers and mobility assistance
• Document all patient interactions thoroughly in compliance with home healthcare regulations and HIPAA standards

Qualifications

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA)
• Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory care
• Knowledge of medical terminology, medication administration, and vital signs monitoring
