Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support.
• Monitor and record vital signs such as blood pressure, temperature, pulse, and respiration rates with accuracy and attention to detail.
• Support patients with nutritional needs by assisting with feeding and ensuring proper hydration.
• Maintain a clean and safe environment by adhering to infection control protocols and safety guidelines.
• Document patient care activities accurately using electronic health record (EHR) systems while respecting patient confidentiality in compliance with HIPAA regulations.
• Collaborate effectively with nursing staff and other healthcare professionals to coordinate patient care plans.
• Use specialized equipment such as Hoyer lifts or other manual handling tools to assist with patient transfers safely.
• Recognize and respond promptly to changes in patient condition or behavior, escalating concerns as appropriate.

Experience

• Prior experience as a Certified Nursing Assistant or in a similar caregiving role within long-term care facilities, hospitals, assisted living communities, or home care settings.
• Familiarity with PCC for medical documentation and patient record management.
• Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS) procedures such as CPR.
