Duties

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and manual handling techniques.
• Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately in electronic health record (EHR) systems.
• Support medication administration under supervision, ensuring proper documentation in compliance with HIPAA regulations.
• Provide specialized care for patients with dementia or Alzheimer’s disease, including memory care strategies and behavioral health support.
• Collaborate with nursing staff to implement individualized care plans for patients with complex needs such as critical care, ICU, trauma center, or pediatric cases.
• Maintain aseptic techniques during procedures like catheter care and wound management to prevent infection.
• Assist in patient transfers and positioning using safe lifting practices and equipment like Hoyer lifts.
• Document patient interactions thoroughly in medical records and electronic health records (EHR/Epic/eClinicalWorks), ensuring accuracy and confidentiality.
• Support patients in community-based settings or home environments by delivering compassionate home & community care services.
