Responsibilities

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
• Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring all documentation complies with HIPAA regulations.
• Provide specialized care for individuals with dementia or Alzheimer's disease, ensuring a safe and supportive environment tailored to memory care needs.
• Collaborate with healthcare teams using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to document patient progress and medical records precisely.
• Assist in infection control practices .
• Support patients in by offering compassionate caregiving that respects their dignity and promotes mental well-being.

Qualifications

• Valid CNA certification from an accredited program; prior experience in long-term care, hospital medicine, or acute care settings is highly preferred.
• Demonstrated proficiency with EMR/EHR systems such as PCC helpful.
• Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS) certification.
• Experience working with individuals with disabilities or developmental delays; familiarity with dementia and memory care practices is advantageous.
