Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient observations and vital signs to track health status and report changes promptly
• Administer medications and perform catheter care following prescribed protocols and medical terminology standards
• Support clients with dementia, Alzheimer's, memory care, and end-of-life needs with patience and compassion
• Document medical records accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance
• Provide light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment
• Utilize assistive devices like Hoyer lifts safely during transfers and mobility assistance while practicing heavy lifting techniques when necessary
• Collaborate with family members, healthcare professionals, and other caregivers to deliver comprehensive home & community care

Skills

• Strong patient observation skills to identify changes in health or behavior promptly
