Job Overview

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) or Home Health Aide (HHA) to provide exceptional care to our clients in home and community settings. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Support clients with medication administration and manage medical documentation in compliance with home healthcare regulations and HIPAA standards
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments, including memory care techniques and behavioral management
• Assist with medical procedures such as catheter care, wound care, end-of-life hospice support, and basic life support (BLS) or CPR when necessary
• Support clients with instrumental activities of daily living such as meal preparation, light housekeeping, laundry, cleaning, and transportation to appointments
• Use proper lifting techniques and equipment like Hoyer lifts to ensure safety during patient transfers and mobility assistance
• Collaborate effectively with healthcare teams to ensure continuity of care and accurate medical records management
