Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Support clients with specialized needs such as dementia, Alzheimer’s, hospice, or end-of-life care, ensuring comfort and dignity
• Use assistive devices like Hoyer lifts and perform catheter care safely while adhering to medical terminology protocols
• Prepare nutritious meals, assist with light housekeeping tasks including cleaning and laundry, and ensure a safe living environment
• Document patient progress using EMR (Electronic Medical Records) systems and communicate effectively with healthcare teams

Qualifications

• Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory or dementia care
• Knowledge of home health care practices and hospice or end-of-life care
• Familiarity with medical terminology, medical records management, and home & community care regulations
• Ability to perform heavy lifting when necessary and operate mobility aids such as Hoyer lifts safely
• Strong patient observation skills combined with compassionate caregiving abilities
• Experience working with EMR/EHR systems for accurate documentation and compliance purposes
• Excellent communication skills to coordinate effectively with clients’ families and healthcare providers
