Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary
• Monitor patient observations including vital signs, mental status, and overall health condition; document findings accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Support medication administration following medical documentation protocols and ensuring compliance with HIPAA regulations
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care strategies
• Assist with medical procedures such as catheter care, hospice support, and working with patients with developmental disabilities or disabilities
• Prepare nutritious meals and assist with meal planning while maintaining proper food safety standards; perform light housekeeping tasks including cleaning and laundry to promote a hygienic environment
• Maintain detailed medical records and documentation of patient care activities to ensure continuity of care and compliance with healthcare regulations

Qualifications

• Prior experience in home health care, assisted living facilities, nursing homes, or long-term care environments is highly preferred
