Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs such as blood pressure, pulse, and temperature; document findings accurately using Electronic Health Record (EHR) systems
• Support medication administration following prescribed protocols while maintaining compliance with home healthcare regulations and HIPAA privacy standards
• Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care strategies and behavioral support
• Assist with catheter care, wound care, and hospice/end-of-life support to ensure patient comfort and safety
• Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to maintain a safe living environment
• Operate medical equipment like Hoyer lifts or other assistive devices safely and effectively during patient transfers

Requirements

• Proven experience in assisted living facilities, nursing homes, or home healthcare environments with a focus on geriatrics or long-term care
