Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and dignity
• Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) systems
• Support medication administration following prescribed protocols while ensuring compliance with home healthcare regulations and HIPAA confidentiality standards
• Provide specialized care including Alzheimer's care, dementia care, hospice support, and end-of-life assistance with sensitivity and professionalism
• Aid with mobility and patient transfer methods utilizing equipment such as Hoyer lifts or patient transfer techniques to ensure safety for both client and caregiver
• Perform light housekeeping tasks like cleaning, laundry, meal preparation, and maintaining a safe environment to promote overall well-being
• Communicate effectively with clients and family members to ensure personalized care plans are followed and needs are met promptly

Requirements

• Home Health aide certification
• Proven experience working with individuals with disabilities, developmental delays, or geriatric patients in home or long-term care settings
• Knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) including CPR certification
