Responsibilities

• Provide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature to track patient health status
• Assist with medication administration following prescribed instructions and document all medical records accurately using Electronic Health Record (EHR) systems
• Support patients with specialized care needs such as Alzheimer's care, dementia care, hospice care, and end-of-life care
• Perform patient observation to identify changes in condition or behavior and communicate findings promptly to healthcare professionals
• Assist with medical procedures including catheter care, wound care, and basic life support when necessary
• Support safe patient transfers using equipment like Hoyer lifts or other assistive devices while practicing proper body mechanics and heavy lifting techniques
• Maintain a clean and safe environment by assisting with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring compliance with home & community care standards
