Home Health Aide

Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ health status, recording vital signs and changes in condition accurately in electronic health record (EHR) or medical documentation systems
• Provide companionship and emotional support
• Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment
• Use medical terminology appropriately when documenting patient care and communicating with healthcare teams
• Operate assistive devices like Hoyer lifts safely to transfer patients and prevent injury

Qualifications

• Proven experience in home health care, assisted living, nursing homes, or long-term care environments
• Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation
• Knowledge of dementia care, Alzheimer's care, hospice support, and working with individuals with developmental disabilities or disabilities
• Valid CPR certification
• Ability to perform patient observation and accurately record vital signs such as blood pressure, pulse, temperature, and respiration
• Experience with medical procedures including catheter care, medication administration, and hospice care support preferred
• Strong communication skills to collaborate effectively with healthcare teams while maintaining HIPAA compliance
