Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices
• Monitor and observe patients’ conditions, recording vital signs such as blood pressure, pulse, temperature, and respiration accurately
• Medication reminders and manage medical documentation in compliance with HIPAA regulations and EMR/EHR systems
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory-related conditions through tailored memory care techniques
• Support clients with catheter care, and hospice needs.
• Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and preferences
• Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment to promote overall health and safety

Experience

• Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
• Demonstrated ability to work effectively with individuals with developmental disabilities or disabilities of any kind.
• Hands-on experience using EMR/EHR systems for documentation and patient records management
• Certification in CPR is required; additional certifications such as hospice care training or dementia care are advantageous
• Experience operating medical equipment such as Hoyer lifts.
• Long-term care experience combined with a compassionate attitude ensures success in this rewarding role
