Home Health Aide

Duties

• Provide attentive patient observation to monitor health changes and report findings accurately using Electronic Health Record (EHR) systems
• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Support patients with specialized needs such as Alzheimer’s care, dementia care, hospice support, and working with individuals with developmental disabilities or disabilities
• Prepare nutritious meals and assist with cooking while ensuring dietary restrictions are followed
• Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall health and safety
• Support patients during transfers using Hoyer lifts or other mobility aids while ensuring safety protocols are followed

Experience

• Prior experience in home health or community care environments is highly preferred
• Experience working in assisted living facilities or nursing homes is advantageous
• Familiarity with medical terminology, vital signs monitoring, medication administration, and documentation procedures is essential
