Duties

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ health status, including vital signs and overall condition
• Support medication administration following prescribed instructions and medical documentation protocols
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments
• Help with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment
• Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Assist with transfers using equipment like Hoyer lifts or other mobility aids while ensuring safety standards are upheld
• Support end-of-life and hospice care needs with compassion and respect for patient dignity
• Follow home healthcare regulations and maintain compliance with HIPAA privacy standards
• Collaborate effectively with nursing staff and other healthcare providers to deliver comprehensive patient care
