Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to promote hygiene and comfort.
• Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately.
• Support medication administration following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient privacy.
• Observe and report changes in patient condition, including behavioral or physical symptoms related to dementia, Alzheimer's disease, or other chronic illnesses.
• Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document medical information clearly and efficiently.
• Assist with mobility using equipment such as Hoyer lifts or transfer belts to ensure safe patient movement.
• Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with home & community care standards.
• Provide specialized care for individuals with developmental disabilities, memory care needs, hospice requirements, or those in assisted living or long-term care settings.
• Support patients during medical procedures such as catheter care or wound management when trained and authorized.
• Collaborate with nursing staff and other healthcare professionals to deliver comprehensive patient care tailored to individual needs.
