Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs and reporting changes promptly
• Administer medications and manage medical documentation in compliance with HIPAA regulations
• Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately record care activities
• Provide specialized care such as Alzheimer's, dementia, memory care, hospice, or catheter care as needed
• Support clients with meal preparation, light cleaning, laundry, and home organization to promote a safe living environment
• Use medical equipment like Hoyer lifts and assist with transfers to ensure safety during mobility tasks
• Document patient progress and communicate effectively with healthcare teams to coordinate ongoing care plans

Skills

• Strong patient observation skills to detect changes in health status quickly and accurately
• Experience working in assisted living facilities, nursing homes, or home health environments
• Knowledge of medical terminology and medical documentation standards
