Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
• Monitor and observe patient health status, including vital signs and overall condition
• Administer medications and manage medical documentation in compliance with home healthcare regulations and HIPAA standards
• Support patients with specialized needs such as Alzheimer's care and dementia care
• Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment
• Use medical equipment such as Hoyer lifts safely while adhering to proper protocols
• Collaborate with healthcare professionals to coordinate patient care plans and report any changes in condition promptly

Skills

• Strong patient observation skills with the ability to recognize changes in health status quickly
• Previous assisted living or home health agency experience preferred
• Familiarity with EMR (Electronic Medical Records) systems and medical documentation standards
• Knowledge of medical terminology, medication administration, and vital signs monitoring
• Experience working with geriatric patients, individuals with developmental disabilities or disabilities in general
• Ability to perform heavy lifting safely and operate mobility aids like Hoyer lifts when necessary
