Hospice Aide (CHHA): Per Diem

Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and wheelchairs
• Monitor and document patient observations, vital signs, and changes in condition accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Support medication administration and catheter care following medical guidelines and HIPAA regulations to ensure patient safety and confidentiality
• Provide companionship and emotional support to patients living with dementia, Alzheimer's disease, or other cognitive impairments; assist with memory care activities
• Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a clean and safe environment in accordance with health standards
• Support hospice care plans by working closely with nursing staff to ensure comprehensive patient care and comfort
• Document all patient interactions thoroughly in medical records and communicate any concerns or changes promptly to the healthcare team
