Hospice Certified Nursing Assistant (CNA)

We are seeking a compassionate, dependable Certified Nursing Assistant (CNA) to join our growing hospice team. In this role, you will provide hands-on personal care and emotional support to hospice patients in skilled nursing facilities, assisted living communities, and private residences throughout our service area.

Responsibilities

• Provide personal care including bathing, grooming, dressing, toileting, oral care, and skin care.
• Follow each patient's individualized plan of care.
• Observe, document, and report changes in patient condition to the RN Case Manager.
• Accurately complete visit documentation in a timely manner.
• Maintain infection prevention and safety standards.
• Collaborate with the interdisciplinary hospice team to provide exceptional end-of-life care.
• Support patients and families with dignity, compassion, and respect.

Qualifications

• Current Wisconsin CNA certification.
• Valid driver's license, reliable vehicle, and current automobile insurance.
• Ability and willingness to travel throughout Allegiant Hospice's service area.
• Strong communication and interpersonal skills.
• Hospice, home health, or long-term care experience preferred but not required.
• Willingness to travel between patient locations, including skilled nursing facilities, assisted living communities, and private residences.
