Responsibilities

  • Assist patients with activities of daily living such as bathing, grooming, dressing, and mobility to promote comfort and independence.
  • Monitor and record vital signs accurately, including temperature, pulse, respiration, and blood pressure, ensuring timely communication of changes to the healthcare team.
  • Support medication administration under supervision, following strict protocols and maintaining compliance with HIPAA and safety standards.
  • Provide emotional support and companionship to patients, fostering a peaceful and reassuring environment aligned with hospice philosophy.
  • Document patient care activities thoroughly in electronic health records (EHR/EHR systems), ensuring accuracy and confidentiality of medical documentation.
  • Collaborate with interdisciplinary team members—including nurses, social workers, and chaplains—to coordinate holistic patient care plans.
  • Utilize specialized equipment such as Hoyer lifts safely to assist with patient transfers, adhering to aseptic techniques and safe handling practices.

Skills

  • Proven experience or familiarity with hospice care, geriatrics, or long-term care environments.
